Medical Assistance 1099 Information

The 1099-MISC form, or U.S. Information Return, is a federally required form used to indicate the amount of payments made to a particular Social Security Number (SSN) or Tax Identification Number during a calendar year for tax purposes.

The Office of Medical Assistance Programs is required by law to issue1099 forms to the same SSN or Tax Identification Number for which Medicaid payments were made throughout the previous calendar year.
