The Healthy Pennsylvania Partnership (HPP) is a partnership of stakeholders from across the Commonwealth with an interest in improving the health of all Pennsylvanians. The HPP includes representatives from health care associations, health systems, health and human services organizations, community collaborations, local public health agencies, government agencies, and more.
The goal of the HPP is to bring together many different communities and stakeholders to identify key health challenges in the Commonwealth and work together to solve them.
Within the HPP there are two initiatives: the State Health Assessment (SHA) and the State Health Improvement Plan (SHIP).
- The SHA reports on the health of the state’s population, identifies areas for improvement, factors that impact health outcomes, and assets and resources that can be used to improve population health. The data in the SHA identifies health-related successes and challenges in Pennsylvania and opportunities for improvement. The SHA provides the foundation to develop the SHIP.
- The SHIP is Pennsylvania’s roadmap to improve health over the next five years. The SHIP includes goals, objectives, and strategies to improve identified health challenges. The main goal of the SHIP is to make everyone in Pennsylvania healthier and to make sure that everyone has a fair chance at good health, no matter who they are or where they live.
The HPP is comprised of all SHIP and SHA members, as well as additional stakeholders and partners that represent the interests of communities throughout the Commonwealth.
The HPP meets semi-annually in April and October to discuss and share information and resources on health priorities throughout the Commonwealth, receive updates on the SHIP and SHA, and provide feedback and suggestions for improvement.

We guide plans to improve the health of all Pennsylvanians using evidence, data, and input from a wide range of different communities and stakeholders to inform our processes.
Collaboration
We respectfully partner with members of many different communities and stakeholders to address the root causes of important public health issues and develop plans for collective action.
Inclusion
We promote an environment where individuals share a sense of belonging and practice acceptance and active listening so that we may engage different populations and ideas.
Accountability
We value and respect each other's time, honor individual commitments, maintain open communication, and recognize our responsibility to improving the health of all Pennsylvanians, while being flexible to changing circumstances.
Accessibility
We promote an open dialogue between stakeholders and the public to share information, and data in an easy to understand and engaging way.
Equity
We aim to provide every person, regardless of location, religion, race, ethnicity, sexual orientation, or gender identity and expression, the same opportunities to live their healthiest life and reach their full potential.
